How to add Pre Check in links forms to Cornerstone

 How to add online history form to Cornerstone via e-mail  

When clients schedule an appointment, we will automatically send them an email containing a link to the patient history form. Alternatively, the clients can access the forms online via our website or app. Although filling out this form is not mandatory, it is a valuable time-saving tool for both clients, technicians, and doctors. If the client chooses to complete the form, an email alert will be sent to the front desk from frontdesk@exoticbirdhospital.com. It is crucial for the front desk to keep the pdffiller.com website open and logged in at all times to access the filled forms and process them.

After you receive the email, you must click the word “link” to open the form filled out by the client. Your role in this process is vital, ensuring we have the necessary information for the client’s pet history. (See image below)

The email’s title will always be something like this: ‘A filled document Exam mammal History.docx has been submitted to you with pdfFiller LinkToFill.’

After you click the link and your pdffiller.com account is open, the next page will be the following. (See image below)

Click “Save to MY DOCS” and ensure the software gives you the window “Success.” Next, click the option at the top of the big orange button: “Go to MYDOCS.”

Convert file to Word Document

After you save the file, the page will take you to the most recent files, which is usually at the top of the list. To verify that it is the correct file, click preview, which is an “eye icon” next to the file name. If you click the file name, you will get different options on the right side of the screen. (see image below).

Click the option “convert” under the Export option. This option ensures that the file will change from a PDF to a Word document

After you convert the document to a Word Document, usually by default, you will find the file in your Download folder files. Please create a unique folder on the computer to save all the files in one folder. Also, rename the file with the pet and client names. This way, it will be easy to find if you need it again another day. Now open the Word document and copy the whole form.

 

Adding form to Cornerstone

The final step is to find the pet history and paste the file to the clipboard. Please make sure the clipboard is unlocked before you do so. If the form does not line up after you paste the file, you can drag the file from the corners and fix the size or the table.

Conclusions

While it’s optional for clients to fill out the form, it can significantly save our technicians and doctor’s time. Your role as the front desk staff is crucial in this process. You are responsible for receiving the filled-out form, keeping it on your computer, and converting it into a Word document. Ensuring the file is saved in a unique folder and renamed with the pet and client names for easy access is also part of your responsibility. Additionally, it would help if you made sure the font and size of the Word document are Arial and 11, respectively. Finally, copy and paste the form into the pet history clipboard in Cornerstone.
