Enter user name : bgomez@exoticbirdhospital.com

Password: ” Please ask management for details”

Click the first option in your screen “My Businesses”

Click next to the Logo ” Manage Mobile app & Website”

Click the option Custom Forms (The white square below Manage Features)

Finally, you will see all forms in the App (RX refill, Appointments, Boarding). Use the drop-down in the top-right to filter and select the form you wish to check.

To see details, please make sure you touch the RX or form line with your mouse cursor that you want to open, and you will get 3 dots at the end of the line, click the 3 dots, and you will see the option “Details,” and you will get all the information of the form, which is the same information that you receive in the emails. This site updates automatically every time a client fills out a form

If you want to export and transfer all the forms to an Excel sheet, please click the sidebar “Export” and select the form you want to view, and also select the date of the forms that you want to export
